Adoption Follow-Up Questionnaire

The Craig Animal Shelter staff would like to thank you for adopting a homeless pet. After a 30-day period, all adopters from the Shelter are asked to complete this questionnaire. It is designed to inform our staff of any problems in our adoption process and to make sure you and your new pet are successfully adjusting.
